Reconstruction of expanding tracheoesophageal fistulae in post-radiation therapy patients who undergo total laryngectomy with a bipaddled radial forearm free flap: Report of 8 cases.

چکیده
BACKGROUND Our surgical approach describes a bipaddled radial forearm free flap (RFFF) for closure of chronic tracheoesophageal fistulae (TEF) in patients who underwent total laryngectomy. The desired functional results were achieved. METHODS Eight patients underwent the procedure. The surgical approach includes exposure and resection of the fistula tract, and a bipaddled RFFF transfer. Key surgical maneuvers include: circumferential dissection and mobilization of the trachea; partial sternal resection in select cases; inset of flap's distal paddle into the anterior esophageal wall; and inset of the proximal skin paddle to the posterior tracheal wall and cervical skin. RESULTS Successful reconstruction of all 8 cases was done to restore a normal diet and a widely patent tracheal opening. One patient developed a delayed esophageal stricture, which was successfully managed with home dilation. CONCLUSION Several TEF treatment approaches have been reported. Our 87.5% esophageal lumen preservation success rate, reestablishment of adequate airway, and uncomplicated postoperative courses demonstrates the reliability of this surgical approach. © 2015 Wiley Periodicals, Inc. Primary versus delayed tracheoesophageal puncture for laryngopharyngectomy with free flap reconstruction.
OBJECTIVES To determine whether postoperative complication rates and speech outcomes differ between patients undergoing primary versus secondary tracheoesophageal puncture following total laryngectomy with free flap reconstruction. STUDY DESIGN Retrospective clinical study in a tertiary academic center. METHODS Between November 2004 and June 2010, 137 patients underwent total laryngectomy o...
متن کاملReconstruction of Buccal Mucosa, Upper and Lower Lip Defect Using Free Radial Forearm Flap with Palmaris Longus Tendon: A Case Presentation
Introduction: Advanced lip carcinomas can invade adjacent structures. Performing surgery for these cancers will lead to defects in this anatomically and functionally important area and will cause post-op difficulties such as drooling, speech alterations and aesthetic considerations, if not properly managed. Case Report: In this study, we will introduce a radial forearm free flap with palmari...
متن کاملClosure of post-laryngectomy pharyngocutaneous fistulae
BACKGROUND Closure of salvage laryngectomy defects with vascularized tissue remains controversial. METHODS We evaluate outcomes in patients who required repair of a fistula after attempted primary closure of salvage laryngectomy defect and assess risk factors for persistent fistula. Between 2001 and 2010, 20 patients were treated for pharyngocutaneous fistulae after primary closure of a salva...
متن کاملThe use of a prefabricated radial forearm free flap for closure of a large tracheocutaneous fistula: a case report and review of the literature
INTRODUCTION The closure of complex tracheocutaneous fistulae is a surgical challenge. We describe a staged approach for management of a patient with a large tracheocutaneous fistula in the setting of prior surgery and local radiation therapy. CASE PRESENTATION A 66-year-old Caucasian man who had undergone prior surgery and radiation therapy to the lower neck presented to our hospital for tre...
متن کاملLaryngotracheal reconstruction with a prefabricated fasciocutaneous free flap for recurrent papillary thyroid carcinoma.
BACKGROUND Prefabricated composite free flaps have been described as a technique for reconstruction of laryngotracheal defects. METHODS We present a 74-year-old woman with recurrent papillary thyroid carcinoma (PTC) who was initially treated with thyroidectomy, tracheal resection, and subsequent vocal cord medialization. The patient's recurrent disease was treated with partial laryngectomy an...
